Elderly Disabled Adult Waiver provides various services to eligible individuals in their home who demonstrate an inability or impairment to perform self-care activities such as dressing, grooming, general household task and other activities of daily living. Individuals must be 65 years of age or older, or be determined disabled according to criteria set forth in this waiver and meet Medicaid financial requirements.


Supervised Independent Living the Independent Living program provides services to people with significant disabilities to enhance their ability to live independently, function in their homes and with their families, and to participate in their communities. The IL program also offers a State Personal Care Attendant (PCA) program and a Community and Family Support Program.


The Community Choices Waiver program provides services in the home and in the community to elders or adults with disabilities who qualify. This program does not, by itself or in combination with other OAAS programs, provide support 24 hours a day. To qualify for the PAS/CCW program you must meet Medicaid eligibility, 21 years old or older, and meet Nursing Facility Level of Care.


New Opportunities Waiver is designed to provide Home and Community Based Waiver Services to recipients who otherwise would require the level of care of an Intermediate Care for the Mentally Retarded (ICFs/MR). To qualify for the NOW, a person must be three years old or older, offered a waiver opportunity slot, and meet ALL of the following eligibility criteria to become a NOW waiver recipient.


Long Term Personal Care All Medicaid recipients age 65 or older, or age 21 with disabilities (meets Social Security Administration disability criteria), must meet the medical standards for admission to a nursing facility and imminent risk criteria, and be able to participate in his/her care and direct the services provided by the worker independently or through responsible representative. Applicants must require at least limited assistance with at least one Activity of Daily Living.
